Referrals to the Exercise Referral Service

There are two ways to access our Exercise Referral Service.

You can be referred by a doctor or health professional, or in some cases you can refer yourself. If independent exercise would be a low or medium risk for you, you do not need a doctor to refer you.

The service is available to anybody who:

  • is 18 or older and lives in or around Cambridge
  • is medically stable but has a diagnosed medical condition or disability
  • is ready and agreeable to becoming more active

Read the full list of referral criteria [PDF, 0.2MB] to check whether you need a health professional to refer you to the service. If your long-term medical condition is not listed, you can refer yourself.

If you have had a cardiac event or procedure or have heart disease, your risk to exercise might be higher. As a result, we will need more information about your health before you can engage with the service. We need a member of your cardiology team or your GP to complete a cardiac referral form – contact us to request this form.

If needed, the following health professionals can refer you to the service:

  • doctors or nurses (all specialisms)
  • dietitians, occupational therapists or physiotherapists (not assistants)

Triage form

Whether or not you need a referral, complete our triage form. It will take about 10 minutes to complete. You can complete the triage form before or after being referred by your doctor, if you need to be.

The form will direct you to appropriate support and activities based on your answers. It’s important to be completely honest when you complete the form, to make sure we help you access the services and activities that will benefit you the most.

We can send you additional information about the support and activities recommended for you. Check the box in the form to tell us you want us to do this.

Information for health professionals

The general referral form and cardiac history referral form are available on SystmOne and EMIS.

If you would like to hear more about the Exercise Referral Service, you can register to receive our newsletter. Our Professionals Update will be sent to you no more frequently than once per month. You can unsubscribe at anytime.

Please help us to spread the word about the service by displaying and circulating our promotional materials. We can provide posters, leaflets, digital slides and social media posts – contact us if you want us to send you some.
