You must be referred to our exercise referral service by your doctor or by a nurse, occupational therapist, physiotherapist or dietician.

Speak to your health professional to find out whether you are eligible for referral.
All doctors’ surgeries in Cambridge can refer patients to the service. Some departments at Addenbrooke’s Hospital, Brookfields Hospital, Royal Papworth Hospital and Chesterton Medical Centre can also make referrals.
Eligible reasons for referral include:
- Anxiety or depression illnesses
- Arthritis, osteoporosis, joint replacement rehabilitation or musculoskeletal injuries
- Cancer
- Chronic pain
- Diabetes
- Heart disease, high blood pressure or cardiac conditions
- Long covid or respiratory illness
- Neurological conditions
- Obesity
If you cannot meet with your health professional, you can send them a copy of the exercise referral form and ask them to complete it for you.
Your health professional should return the completed form to you, and you should then contact your preferred branch of the exercise referral service using the contacts below.
We are unable to accept referrals to exercise referral service sessions at Hills Road Sports Centre, Chesterton Sports Centre or for home-based programmes, until spring 2025.
